2005 RMA Summer Guitar Workshop Registration The form below can be used for registration for the 2005 RMA Summer Guitar Workshop. Please register as soon as possible, as class sizes will be limited. Filling out the form below preregisters you for a slot at the workshop. A deposit for 50% of the tuition must then be mailed to: Raleigh Music Academy 4720 Hargrove Rd., Suite 130 Raleigh, NC 27616. There are still a few spaces left for this workshop, but you will need to enroll and pay the tuition as soon as possible to secure a spot. Cashier checks, money orders and personal checks are acceptable forms of payment. If you would like to pay via Paypal, please send your payment to paul@raleighmusicacademy.com. NOTE: All participants in the workshop must add a non-refundable $25.00 registration/materials fee to the total of the package they choose. Enrollment is limited for this workshop and it is therefore necessary that all registrants do their best to attend. For any cancellations after June 1st, there will be forfeiture of 50% of the tuition (i.e. the deposit), as it will be very difficult to fill a spot at that late of a date. After registering, you will receive a verification email with the total due for the package selected. Your deposit for the total of 50% of your total is due as soon as you register with the balance due by May 15th.
